Home Improvement Salesperson (HIS) License Application Steps

READ CAREFULLY

1. Print the Application Below. Your signature and Social Security must be filled in by hand with ink.

• Full legal names are required. If you do not have a middle name, write “None” or “No Middle Name” in the space provided. If you have only an initial for your first or middle name, write “(Initial Only)” after the initial.

• Leave no space blank. Write “none” for HIS fax number

• You must provide a mailing address where you will be able to receive mail for up to 60 days. PO boxes are not accepted.

• For section two of the application, if you have ever been convicted of a crime you should answer “Yes” to the criminal conviction question on the application and provide a detailed explanation of the circumstances resulting in your conviction on the Disclosure Form below. Applicants with criminal convictions are not automatically denied licensure – each application is reviewed individually based on the applicable sections of law. When reviewing criminal convictions, CSLB considers factors such as the nature and severity of the crimes, the amount of time that has passed since the convictions, and any evidence of rehabilitation submitted by the applicant.

• Fill out all questions truthfully! Spending a few extra minutes now may save you weeks or months later!

2. Write a check/cashiers check for $200 made payable to the Contractors State License Board

3. Send the completed application with a check by mail to:

Within one (1) week you will receive a letter by mail stating that the CSLB has received your application. The document will show the Application Fee Number. Take a picture of this form and email it to onboarding@gogenow.com or use the secure form above. You can check the status of your application by entering your Application Fee Number here:

Within three (3) weeks you will receive a manilla envelope by mail. The envelope will contain three LiveScan forms. All three forms must be filled out. You will need to bring the forms to a LiveScan location (https://oag.ca.gov/fingerprints/locations) to be fingerprinted. Note: Fingerprinting will typically cost between $60 - $80. Once completed, the LiveScan operator will stamp each form and keep one copy. One copy must be mailed back to the CSLB immediately using the above address. Take a clear image of the third copy and email it to onboarding@gogenow.com or upload it to the secure form above.

The final approval may take up to five (5) weeks. If your application is denied, you will receive notice of the rejected application and may have the opportunity to cancel the original and resubmit a new application with more information. If you resubmit within 90 days you will not have to get fingerprinted again.
